I consent to the collection, use and disclosure of my personal information (such as name, phone number and address) by Geogian Bay Mobile Veterinary Services in accordance with the purposes set out in the Personal Informantion Protection and Electronic Documents Act These purposes include:

Maintaing accurate and complete client/patient files in compliance with the regulations of the College of Veterinarians of Ontario (CVO)
Providing clients with foods and services which include contacting clients with regards to their pet's heatlth, follow up on patient treatment and care. And billing for items and services provided.

Maintaing communications with third parties involved in the provision of Veterrinary care and services including other Veterinary hospitals/clinics/offices and Veterinary insurance providers.
